If you plan to bill your PPO insurance for out of network care, I can provide a super bill for you to submit to your insurance company. Every insurance plan is different and questions about coverage should be asked directly to your insurance company. Please let me know your intentions to bill insurance during our first session and we can discuss in more detail.
